State who was involved, what was offered or demanded, dates and locations, and how it relates to evaluation or business awards. Attach evidence (messages, transfers, receipts) if you can.

List concrete incidents with dates, places, and witnesses. Describe how your work was affected.
- Example: insulting remarks in a meeting on 25 June
- Example: repeated demeaning language or isolation from normal duties
- Example: seating or assignments changed to exclude you without explanation

Conflicts of interest: side jobs, misuse of position, related-party deals, misuse of inside information
Explain the relationship or transaction, roles of the people involved, and why it may violate policy or law. Name others who can corroborate if known.

Improper work conduct: false reporting, manipulation, misuse of assets or data
Describe the incident timeline, who was informed, what was reported upstream, and what was concealed. List witnesses.

Explain the suspected breach (e.g. altered certificates, unsafe practice), approximate period, product or service, and known contacts on both sides.
